Medicare, Medicaid and the Children’s Health Insurance Program (CHIP) comprise almost a quarter of all federal outlays in 2023: Medicare accounts for 13 percent of outlays, while Medicaid and CHIP together account for another 10 percent. This presents a significant challenge, as the Congressional Budget Office (CBO) projects that over the next decade both Medicare and Medicaid will grow faster than the economy as a whole, effectively crowding out other programs and national policy priorities.

Misinformation During COVID: How Should Nurse Practitioners Respond?

Highlights

Misinformation has compounded the COVID-19 pandemic health care crisis.

Previous research on misinformation exists and can aid in combatting the current misinformation crisis.

Misinformation is attractive because it offers absolutes, while science is more ambiguous.

Professional medical organizations have called on social media sites to quell the spread of misinformation.

Nurse practitioners can address misinformation online and at the bedside by leveraging the patient–provider relationship.

Empathy and trust may be the best way to sway those who believe in misinformation.

"The dawning of 2020 brought a fresh perspective and vision for the new year and decade. The American Association of Nurse Practitioners outlined important actions for the future practice of nurse practitioners (NPs) in its 2020 State Policy Priorities.1 Current progress was highlighted as the adoption of full-practice authority laws for NPs in states across the country, increased visibility, and broadening acceptance from insurance companies for treatment during the previous decade.2 Across the country, NPs continue to make strides toward greater recognition and ability to practice to their fullest scope; however, individual states continue to be a battleground for the ability to practice independently or with tiered levels of supervision.

On a national scale, several prophysician groups have raised their social media presence verbalizing negative biases toward NPs. Additionally, in a June 2020 letter to the Centers for Medicare and Medicaid Services, the American Psychological Association presented opinions recommending psychiatric NPs’ return to psychiatrist supervision after the pandemic.3 The American Academy of Emergency Medicine modified their position statements in February 2020 and September 2020, noting NPs must avoid using terms fellowships or residency related to NP training, advising NPs must have physician supervision, and recommending NPs not supervise medical residents nor interfere with residents’ clinical opportunities."

In 2020, Florida and California passed legislation to relax scope-of-practice laws for nurse practitioners (NPs). Florida's new law went into effect July 1 and allowed NPs to practice independently, after completing 3000 hours of practice supervised by a physician in the past 5 years. The bill in California, which will take effect in January 2023, requires NPs to work
under the supervision of a physician for 3 years before practicing independently.  

Florida and California join 12 other states that require similar transition periods before allowing NPs to practice independently,  and 14 others that allow full practice authority. Previously, states that expanded scope-of-practice laws tended to have more rural  patient populations.

Currently, 22 states continue to have scope-of-practice laws that restrict NPs from practicing independently, though one, Massachusetts, is exempting advanced practice registered nurses who have completed 2 years of supervised practice from the state's requirement for physician supervision during the current state of emergency because of COVID-19.
